Growth hormone released from the pituitary stimulates the liver and other tissues to produce insulin-like growth factor 1, a stable circulating protein that serves as a practical marker of activity. Clinical studies therefore track IGF-1 concentrations alongside the hormone itself, and they commonly measure body composition with imaging rather than relying on body weight alone. Visceral adipose tissue, the fat surrounding abdominal organs, is quantified by computed tomography in the studies that supported approval. Adverse effects reported in trials include injection-site reactions, joint pain, and increases in blood glucose, which is why monitoring accompanies use.
Questions remain about how much of the observed fat reduction reflects direct GHRH-receptor signaling versus the downstream growth hormone and IGF-1 surge. It is also unclear whether the compound produces meaningful benefit in populations without lipodystrophy, since trials in cognitive impairment did not reach their stated goals. Long-term effects on glucose metabolism and on cardiovascular outcomes are not fully characterized. Published work generally describes effects on surrogate markers rather than on hard clinical endpoints, and independent replication of some findings is limited.
The peptide is synthesized chemically rather than extracted from biological sources. Solid-phase synthesis builds the chain from the C-terminus toward the N-terminus, after which the hexenoyl group is attached. Purity is typically assessed by high-performance liquid chromatography, and identity is confirmed by mass spectrometry. Regulatory review of the finished product focuses on these analytical controls, since small deviations in sequence or modification can change biological activity. Questions about long-term effects on the pituitary axis remain areas of continued investigation.
Tesamorelin is a synthetic peptide that belongs to the growth hormone-releasing hormone (GHRH) family. Its sequence corresponds to the fully active 44-amino-acid form of human GHRH, with a single structural modification: the addition of a trans-3-hexenoyl group at the N-terminus. That modification is not found in the naturally occurring hormone and was introduced deliberately during development to improve stability against enzymatic degradation. The compound is therefore best described as a stabilized analogue rather than a naturally occurring peptide.

| Research status | Investigational outside the approved indication | Trials in cognitive impairment did not meet primary endpoints |
Identity and purity are judged through a combination of chromatographic and mass spectrometric techniques. Reversed-phase high-performance liquid chromatography separates the intact peptide from truncated, oxidized, and deamidated variants, and the resulting peak-area percentages yield a purity figure. Electrospray ionization mass spectrometry confirms the expected molecular mass and can expose unanticipated modifications. Amino acid analysis and peptide mapping support sequence fidelity, while water content, pH, sterility, and bacterial endotoxin testing describe the physical and microbiological attributes of a finished lot.
Regulatory position depends on jurisdiction and on the form in which the material is sold. A branded product holds approval in the United States for a defined indication, and prescribing is confined to that label. Material marketed for laboratory research is not evaluated for human use and carries no such clearance. Independent verification therefore rests on certificates of analysis, third-party testing, and documented chain of custody. The substance also appears on the World Anti-Doping Agency prohibited list within the category covering growth hormone-releasing factors.
Lyophilized material is typically held under refrigeration between two and eight degrees Celsius, shielded from light and ambient moisture. Peptides of this size adsorb to glass and plastic, so working procedures often call for low-binding containers and as few transfers as possible. Absorbed water during weighing shifts the apparent mass of a sample, and controlling room humidity reduces that source of error. Once dissolved, solutions are kept cold and used within the interval printed on the accompanying label or certificate. Degradation accelerates markedly in dilute aqueous form.
Tesamorelin is a synthetic analog of growth hormone-releasing hormone, a peptide hormone produced by the hypothalamus. The molecule retains the 44-amino-acid sequence of human GHRH and carries a trans-3-hexenoyl modification at its N-terminus. This modification increases resistance to enzymatic degradation and extends the peptide's functional stability relative to native GHRH. The compound is supplied as a lyophilized powder for reconstitution and subcutaneous administration in clinical settings. Its development code was TH9507, and it belongs to the GHRH analog class. It is not a growth hormone product; instead, it acts upstream to stimulate endogenous growth hormone release.
Clinical interest in tesamorelin arose from the need to address visceral adiposity in people living with HIV. Antiretroviral therapy improved survival but was associated in some patients with central fat accumulation, altered lipid profiles, and metabolic complications. This condition, often called HIV-associated lipodystrophy, involves excess visceral adipose tissue that is difficult to manage through diet and exercise alone. Investigators evaluated tesamorelin because GHRH analogs can stimulate growth hormone secretion and influence fat distribution without direct liposuction or invasive procedures.
A Phase 3 program led to regulatory approval in the United States in 2010 for reduction of excess visceral abdominal fat in adults with HIV and lipodystrophy. Subsequent studies examined effects on liver fat, muscle area, and metabolic markers, with mixed findings for some endpoints. Long-term cardiovascular outcomes and effects on mortality remain uncertain because most trials were relatively short and focused on imaging-based fat measurements. Use in populations without HIV has been studied experimentally but is not part of the approved indication.
By the second half of the 1800s, the scientific study of psychology was becoming well established in university laboratories. Although there were a few scattered voices calling for applied psychology, the general field looked down upon this idea and insisted on "pure" science as the only respectable practice. This changed when Lightner Witmer (1867–1956), a past student of Wundt and head of the psychology department at the University of Pennsylvania, agreed to treat a young boy who had trouble with spelling. His successful treatment was soon to lead to Witmer's opening of the first psychological clinic at Penn in 1896, dedicated to helping children with learning disabilities. Ten years later in 1907, Witmer was to found the first journal of this new field, The Psychological Clinic, where he coined the term "clinical psychology", defined as "the study of individuals, by observation or experimentation, with the intention of promoting change". The field was slow to follow Witmer's example, but by 1914, there were 26 similar clinics in the US. Even as clinical psychology was growing, working with issues of serious mental distress remained the domain of psychiatrists and neurologists. However, clinical psychologists continued to make inroads into this area due to their increasing skill at psychological assessment. Psychologists' reputation as assessment experts became solidified during World War I with the development of two intelligence tests, Army Alpha and Army Beta (testing verbal and nonverbal skills, respectively), which could be used with large groups of recruits.
==== Shotgun sequencing ==== High throughput shotgun sequencing using tools such as Solexa or Illumina yields approximately 5 million sequence tags per sample of maternal serum. Aneuploid pregnancies such as trisomy were identified when testing at the fourteenth week of gestation. Fetal whole of genome mapping by parental haplotype analysis was completed using sequencing of cffDNA from maternal serum. Pregnant females were studied using a 2-plex massively parallel maternal plasma DNA sequencing and trisomy was diagnosed with z-score greater than 3. The sequencing gave sensitivity of 100 percent, specificity of 97.9 percent, a positive predictive value of 96.6 percent and a negative predictive value of 100 percent.
Think tanks have been important allies for United States presidents since the Reagan administration, writing and suggesting policies to implement, and providing staff for the administration. For recent conservative presidents, think tanks such as The Heritage Foundation, the Hoover Institution, and the American Enterprise Institute (AEI) were closely associated with the Reagan administration. The George H. W. Bush administration worked closely with AEI, and the George W. Bush administration worked closely with AEI and the Hoover Institution. The Trump administration works closely with the Heritage Foundation. For recent liberal presidents, the Progressive Policy Institute and its parent the Democratic Leadership Council were closely associated with the Clinton administration, and the Center for American Progress was closely associated with the Obama and Biden administrations. Think tanks help shape both foreign and domestic policy. They receive funding from private donors, and members of private organizations. By 2013, the largest 21 think tanks in the US spent more than US$1 billion per year. Think tanks may feel more free to propose and debate controversial ideas than people within government. The progressive media watchdog Fairness and Accuracy in Reporting (FAIR) has identified the top 25 think tanks by media citations, noting that from 2006 to 2007 the number of citations declined 17%. The FAIR report reveals the ideological breakdown of the citations: 37% conservative, 47% centrist, and 16% liberal.

=== In television === "Cancelled", an episode of the animated sitcom South Park Series 2 of COBRA, a British thriller series, revolves around a sustained campaign of cyberwar against the United Kingdom and the British government's response to it.
The IDF's mission is to "defend the existence, territorial integrity and sovereignty of the state of Israel. To protect the inhabitants of Israel and to combat all forms of terrorism which threaten the daily life." The Israeli military's primary principles derive from Israel's need to combat numerically superior opponents. One such principle, is the concept that Israel cannot afford to lose a single war. The IDF believes that this is possible if it can rapidly mobilize troops to insure that they engage the enemy in enemy territory. In the 21st century, various nonconventional threats including terrorist organizations, subterranean infrastructure operated by Hamas, etc. have forced the IDF to modify its official defense doctrine.
The first ENSO pattern to be recognised, called Eastern Pacific (EP) ENSO, to distinguish it from others, involves temperature anomalies in the eastern Pacific. In the 1990s and 2000s, variations of ENSO conditions were observed, in which the usual place of the temperature anomaly (Niño 1 and 2) is not affected, but an anomaly also arises in the central Pacific (Niño 3.4). The phenomenon is called Central Pacific (CP) ENSO, "dateline" ENSO (because the anomaly arises near the dateline), or ENSO "Modoki" (Modoki is Japanese for "similar, but different"). There are variations of ENSO additional to the EP and CP types, and some scientists argue that ENSO exists as a continuum, often with hybrid types. The effects of the CP ENSO are different from those of the EP ENSO. The El Niño Modoki is associated with more hurricanes more frequently making landfall in the Atlantic. La Niña Modoki leads to a rainfall increase over northwestern Australia and northern Murray–Darling basin, rather than over the eastern portion of the country as in a conventional EP La Niña. Also, La Niña Modoki increases the frequency of cyclonic storms over Bay of Bengal, but decreases the occurrence of severe storms in the Indian Ocean overall. The first recorded El Niño that originated in the central Pacific and moved toward the east was in 1986. Recent Central Pacific El Niños happened in 1986–87, 1991–92, 1994–95, 2002–03, 2004–05 and 2009–10. There were "Modoki" events in 1957–59, 1963–64, 1965–66, 1968–70, 1977–78 and 1979–80.
== Applications == Hafnium isomers (mainly 178m2Hf) have been considered as weapons that could be used to circumvent the Nuclear Non-Proliferation Treaty, since it is claimed that they can be induced to emit very strong gamma radiation. This claim is generally discounted. DARPA had a program to investigate this use of both nuclear isomers. The potential to trigger an abrupt release of energy from nuclear isotopes, a prerequisite to their use in such weapons, is disputed. Nonetheless a 12-member Hafnium Isomer Production Panel (HIPP) was created in 2003 to assess means of mass-producing the isotope. Technetium isomers 99m43Tc (with a half-life of 6.01 hours) and 95m43Tc (with a half-life of 61 days) are used in medical and industrial applications.

=== Controversy === Use of this class of drugs has a history of criticism in residential care. As the drugs used can make patients calmer and more compliant, critics claim that the drugs can be overused. Outside doctors can feel under pressure from care home staff. In an official review commissioned by UK government ministers it was reported that the needless use of antipsychotic medication in dementia care was widespread and was linked to 1800 deaths per year. In the US, the government has initiated legal action against the pharmaceutical company Johnson & Johnson for allegedly paying kickbacks to Omnicare to promote its antipsychotic risperidone (Risperdal) in nursing homes. There has also been controversy about the role of pharmaceutical companies in marketing and promoting antipsychotics, including allegations of downplaying or covering up adverse effects, expanding the number of conditions or illegally promoting off-label usage; influencing drug trials (or their publication) to try to show that the expensive and profitable newer atypicals were superior to the older cheaper typicals that were out of patent. Following charges of illegal marketing, settlements by two large pharmaceutical companies in the US set records for the largest criminal fines ever imposed on corporations. One case involved Eli Lilly and Company's antipsychotic Zyprexa, and the other involved Bextra. In the Bextra case, the government also charged Pfizer with illegally marketing another antipsychotic, Geodon.
Thiotepa, as well as its more reactive metabolite, tepa, work as an alkylating agent via its aziridine ring. Due to the basic nature of aziridine and the physiological pH, aziridine is protonated to form the aziridinium ion, resulting in an electrophilic moiety that is highly susceptible to nucleophiles. DNA reacts through the nucleophilic N-7 position of guanine onto the electrophilic aziridine ring, rendering alkylated nucleobases. Thiotepa contains three reactive aziridine rings, allowing a single molecule to alkylate multiple nucleobases. Hence, it is a polyfunctional alkylating agent. This property also gives rise to its ability to cross-link DNA strands. Apart from its mechanism of action, it is suggested that thiotepa can function as a prodrug. Due to its moderate lipophilicity, it first penetrates the cell membrane, followed by hydrolysis to release the more hydrophilic aziridine ring. The aziridine ring can once again alkylate the DNA. The highly reactive metabolite tepa can be considered as an active metabolite and alkylates DNA similar to its parent drug. Ultimately, the alkylation of DNA leads to cell damage and can lead to cell death. Cross-linking blocks the separation of DNA strands, inhibiting replication and the proliferation of cells.
Protein efficiency ratio (PER) is the ratio of weight gain to the amount of protein ingested. It is usually tested with rats. Biological value (BV) essentially estimates the proportion of food nitrogen kept in the body by subtracting out nitrogen found in urine and feces. Nitrogen is assumed to originate from protein. Net protein utilization (NPU) is similar, except it only subtracts out urine nitrogen. They are methods based on nitrogen balance. Modern measurements analyze two separate aspects: protein digestibility and amino acid balance. The former is measured by comparing how much protein is found in the food before and after it goes through the digestive tract or a part of it. The latter is measured by taking the amino acid profile of a protein and comparing it to essential amino acid needs of an organism, typically humans.

It acts upstream at the pituitary receptor and depends on functioning somatotroph cells to produce any effect. Growth hormone injections bypass that step and deliver the hormone directly. The pharmacokinetic profiles and the resulting feedback on the body's own secretion therefore differ.
Insulin-like growth factor 1 is a downstream product of growth hormone action and changes more slowly than the hormone itself. Its blood concentration is used as an integrated indicator of whether the pathway has been stimulated. Interpretation requires attention to nutrition, illness, and other factors that shift IGF-1 independently.
No. The approved indication concerns excess visceral abdominal fat in adults with HIV infection and lipodystrophy, a specific clinical population. It is not cleared for general weight reduction or for cosmetic use. Studies in other groups remain investigational.
It shares the 44-residue sequence of human GHRH but carries an added trans-3-hexenoyl group at its N-terminus. That addition does not occur in the natural hormone and serves mainly to resist enzymatic breakdown. The receptor target and signaling pathway remain the same.
